I was taken to the hospital by ambulance and need a copy of the report. How do I get a copy?

The laws regarding privacy of medical records (HIPPA) are very strict. 


You may receive a copy of your own report for no charge. Parents or guardians of minors may receive a copy of their report at no charge. A copy of your photo ID is required to receive your copy. Requests by Law Offices or insurance companies require a copy of a signed release by the patient. 


To secure a copy, contact the Fire Chief at 708-755-9589 or by email. There is a $25 administrative charge for these records. It is all handled through the Chief’s office.
